The process for a new patient can seem somewhat overwhelming so we want to make this experience as easy for you as possible. The checklist below will help you prepare for your visit to our office.

For my first visit, I need to:

• Check in 20 minutes prior to appointment time
• Obtain directions to our office prior to appointment
• Bring completed New Patient Paperwork
• Bring Insurance Card and Driver’s License or Picture ID
• Bring any pertinent medical records that will be required for your visit
• Bring Co-Pay required by your insurance

The forms are securely submitted to our office. Please fill them out completely and click the Submit button when finished.

For directions and a map please see the Office Information tab. As a reminder, we are located on the North Kansas City Hospital Campus. Due to the size of the campus and our office being located on the 12th floor please allow enough travel time for parking and elevators to be in our office 20 minutes prior to your scheduled appointment time.
